I think the cameraman was mean to catch the poor lad unawares before he had finished dressing. Standards today!
Actually, I think the image is saying, I have to take a dump but I am no longer flexible enough to remove my own pants (or wipe.)
Note the quizzical look on his face as he wonders if you will help him with that, especially the latter.
In an unrelated subject, I was looking for my press photo and did a google search. Sometimes google fails in its search, and other times it fails spectacularly!
